PCC respects the privacy of participants, families, employees, providers and website visitors. We use administrative, technical and physical safeguards to protect confidential information as required by federal and Pennsylvania law, our contracts and PCC policy.
Participant health and personal information is handled differently from ordinary website information. This page explains how to read our Notice of Privacy Practices, use your privacy rights, and reach PCC’s Privacy Officer. Information collected through this website is covered by our separate Website Privacy Policy.

Our Notice of Privacy Practices explains how PCC may use and share your health information, your privacy rights, and our responsibilities to protect your information.

Your Privacy Rights
Subject to the limits allowed by law, you have the right to:
- See and get a copy of your PCC record.
- Ask us to correct information you believe is wrong or incomplete.
- Ask us to limit how we use or share your information.
- Ask us to contact you in a confidential way, such as at a different phone number or address.
- Get a list of certain disclosures we have made of your information.
- Get a paper copy of our Notice of Privacy Practices.
- Choose someone, such as a legal guardian or person with health care power of attorney, to act for you.
Request Privacy Information
To request a copy of your record, a correction, a list of disclosures, a restriction, confidential communications, or other privacy information, contact PCC’s Privacy Officer, Alyssa Gallone, RN, BSN.

Some requests must be made in writing. If so, we will send you the form and help you complete it. We will respond to requests to see or copy your record within 30 days, as required by federal law.
Report a Privacy Concern
If you believe your privacy rights were violated or your information was shared improperly, please tell us. Contact PCC’s Privacy Officer, Alyssa Gallone, RN, BSN, at 215-725-7200, ext. 1050 (TTY 711), by mail at the address above, or by email at agallone@patientcarecoordination.org. PCC will look into your concern and respond.
You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, at 1-800-368-1019 (TDD 1-800-537-7697) or online at www.hhs.gov/hipaa/filing-a-complaint.
PCC will not retaliate against you for filing a privacy complaint.
