Important Service Update: Please note that PCC does not currently provide active Nursing Home Transition (NHT) services. We maintain this page strictly as an educational resource because we believe transition planning remains a vital topic for individuals exploring community living.
Pennsylvania’s Office of Long-Term Living (OLTL) oversees programs and services that support eligible older adults and individuals with physical disabilities, including services that may support community living and transitions from institutional settings.
For some individuals living in a nursing facility, returning to a home or community setting may be possible when needs can be safely supported and the individual wishes to explore community living. Transition planning can involve housing, home accessibility, personal assistance services, medical and behavioral health supports, transportation, durable medical equipment, benefits and other community resources.

If you are a PCC participant and want to discuss a change in living arrangement or community transition goal, speak with your assigned coordinator. Your coordinator can help identify questions, communicate with appropriate partners and locate available resources within the rules of your program.
