PCC is a contracted service coordination provider with the Managed Care Organizations (UPMC, PA Health and Wellness and Keystone First) under the new Community HealthChoices (CHC) waiver that replaced the previous waivers for participants in Philadelphia, Bucks, Chester, Delaware and Montgomery counties on 1/1/19 and will replace waivers for all other counties in Pennsylvania 1/1/20.

Waivers that transitioned to CHC
- Aging Waiver.
- Attendant Care Waiver participants ages 21 and older; participants under 21 will transition to the OBRA Waiver.
- Independence Waiver participants ages 21 and older; participants under 21 will transition to the OBRA Waiver.
- Phase 2 OBRA Waiver participants ages 21 and older who are nursing facility clinically eligible; participants under 21 or not nursing facility clinically eligible will remain in OBRA.
Enroll CHC offers information for those currently enrolled in CHC, those who will be enrolling in 2020 and participants who are newly approved for waiver services in a PA county that already transitioned to CHC. Enroll CHC will also allow you to change your managed care health plan at any time amongst the three health plans (Keystone First, PA Health and Wellness, and UPMC).
